By submitting this form, I am giving permission for my team to participate in the Blue Illusion Competition. I understand that in any athletic activities, including dancing, accidents can happen. In that event that my child is injured, I release the Davenport School District, Central High School, the team director, and the staff from any and all liabilities. I understand that my team will be dancing in a safe environment and that all precautions will be taken to ensure the safety of my team.
